:: Equipment Reservation and Checkout Policies ::

Faculty/Staff Check-out / Reservations Procedure
  • Faculty and staff may make reservations for equipment over the phone.
  • Faculty/staff ID must be presented at time of check-out.
Faculty/Staff Authorizing Student Equipment Check-out
  • Faculty may authorize students in the following ways:
    • Submit a class list with the your signature indicating course and necessary equipment;
    • Sign Equipment Request Form indicating course, necessary equipment, date(s) needed,and the name(s) of students authorized.
Student Check-out / Reservations Procedure
  • All equipment orders must be requested in person at the MDS office, no reservations over the phone.
  • Students must present a current Cal Poly ID.
  • Equipment check out for class use requires a faculty authorization, which can be obtained in the following ways:
    • Class list submitted by instructor on file with MDS;
    • Submit a faculty signed Equipment Request Form for the required equipment and period of use.
Equipment Rental Information
  • Equipment checked out for personal use is subject to Equipment Rental Rates. Rental fees must be paid by check or cash at time of check-out.
  • All ASI organizations/clubs are subject to Equipment Rental Rates. Clubs that are considered ASI Academic Clubs, affiliated with an academic department, are not subject to a rental charge. Academic Clubs must comply with the Student Check-out Procedure and have a faculty authorization signature.
Reservations
  • Items reserved and not picked up three hours after the reservation time are subject to being released to another user.
  • If circumstances prevent the customer from picking up the reserved item at the specified time, he/she can call MDS (756-7198) to hold it for pick up at a later time, subject to availability.
Use of Equipment
  • MDS equipment may be used, without rental fees, for course related/instructional use, academic, and State-funded departments, colleges, and divisions. C.A.M. prevents the use of State equipment for any purpose other than the performance of official Cal Poly business.
  • In general, there is a three (3) day at a time maximum check out period with the week-end counting as one (1) day of checkout.
  • Equipment may be checked out for a rental fee for personal, ASI, club or Foundation use, as long as they are a current Cal Poly faculty, staff or student.

Late Fees
  • Late fees will be charged on all over due items.
  • Late charges on high demand items are $15 per day per item. These high demand items include all laptop computers, data/video projectors, digital cameras, digital camcorders.
  • Late charges on general items may be up to $10 per day per order.

Damaged, Lost or Stolen Equipment
  • Any damaged or missing equipment/accessories not noted at check-out will be charged to the customer.
  • Lost or stolen equipment will be billed at the current replacement cost.
  • MDS is not responsible for damage to customers personal equipment used in conjunction with MDS equipment.
Holds
  • If a late fee or missing/damaged equipment charge is not paid in 2 weeks OR payment schedule worked out with MDS office, a hold on records and registration will be placed on the students record.
